So I finally got around to finding a p PKMN p in Yellow through the Ditto trick. I'm a little confused as to how it works, however. When I battled it, the fifth item in my Pack (Master Ball) turned into a Thunderstone. Is it possible to get glitch items (I'm most interested in the surfboard item) this way, and if so, which item do I have to have in the fifth slot for this to happen? Thanks.

p PKMN p adds $20 to every hex that isn't $2X $3X $6X $7X $AX $BX $EX $FX. Additionally, you'll only be able mutate items that aren't these hexes. The BIG LIST will help you to get the desired item you need. Note that the glitch items won't be exactly as the big list shows since glitch items are different in Yellow and the biglist was made before Yellow items were documented, but they will still be glitch items. However, you will not be able to get the surfboard unless you use the Glitch Pokemon EB, EE, F3, or F6 and have a Great Ball in your 3rd item slot.

Edit: NINJA'd It's possible to obtain a few glitch items with p PKMN p (C2) alone, but this doesn't include the surfboard: ????? (07) unfortunately. p PKMN p increases the item identifier by $20 (see here and won't mutate the fifth item if the item is already $2X $3X $6X $7X $AX $BX $EX $FX (See GARY 'M 9's post for more information what will mutate and what doesn't). You can use the Johto guard glitch (combined with the Cable Club blackout glitch) to encounter other item mutation glitch Pokémon in Yellow, and, in Red/Blue, by encountering an F2 ('Chisai-u [3]') with a Bicycle in the 5th position we can turn the Bicycle (06) into the surfboard ????? (07). In Red/Blue, Paco also found that you can also encounter hex:F1 '94' via the Old Man glitch with Professor Oak (who will have a 94) provided that the last Trainer fought was a certain Rocket member in Silph Co, but 94 cannot get the surfboard. I noticed that you commented on the video, but I'll post it here for everyone else to clarify which Trainer it is: I suppose that the Old Man glitch Trainer rosters are loaded as the roster number corresponding to the last Trainer [D05D], which would explain Professor Oak's glitch team because he only has three rosters, but don't quote me on that. If we force e.g. Professor Oak to send out his 4th roster via the Ditto glitch, it will come up as a Scientist's first roster, because Chief comes next (which actually has no roster) and then Scientist. So all rosters after Lance's first and only roster are glitch teams, as well as all roster's after Agatha's 'second' roster (Lance #1), etc. There are lots of Team Rocket members, so maybe that's why some of the later ones yield a glitch team? With p PKMN p alone:Please note that this list is incomplete. Unfortunately, I couldn't find secondary data about the Yellow glitch item effects, so I'm having to do this from experimentation. Using Burn Heal (0C) gives: "?????" (2C): This a second ????? item, however this one gives you Prof. Oak's "This isn't the time to use this!" message and presumably does nothing :(. Using Gold Teeth (40) gives: "11F" (60): Hangs the game. Using X Attack (41) gives: "B4F" (61): Nothing happens, apparently but we don't get Oak's unusable message. Using X Defend (42) gives: "w m" (62): Freezes the game. Using X Speed (43) gives: "ws m" (63): Freezes the game.

In Yellow when I verified the item mutation with C2 via Ditto trick, I saw a interesting and useful glitch item. p PKMN p turned my Super Rod (4E) into a Lg - (6E), this glitch item can restart a trainer battle from the beginning (e.g is useful for defeat a lot times to Lance´s Gyarados in a single battle and win a lot exp points with their respective Evs).
Is recommended not use to Lg - Glitch Item outside of battle because cause glitches in the area where is used.

This item might be even more useful in conjunction with the cable trade blackout glitch. For example, if you set up a trade where the other user has at least 5 low level Mews then, after blacking out and switching your pokemon party, you surf at Cinnabar Island in R/B (or battle the elite 4 in Yellow), you could get the experience from each Mew before resetting the battle. This would work with any low level pokemon you want of course. The only annoying part is that the first pokemon in your opponent's party will have excessive HP (in my experience at least) so it's probably good to keep a high level pokemon in the team to knock it out faster (or one with a one-hit KO move). It would be nice if there were a way around fighting the one pokemon with excessive HP. As I understand, effort points have nothing to do with your opponent's level so this could mean you'd be able to max out your EVs at a very low level (useful for Pokemon Stadium or Colloseum2 battles in Yellow). BTW, I haven't tried this with LG - (6E) yet, so I'm mostly just speculating... Is LG - (6E) available in R/B?

Is LG - (6E) available in R/B? Lg - (6E) is not available in Red/Blue via F1, F2 or F9 Item Mutate Glitch Pokémon. If you get a Lg - via Gameshark, in Red/blue apparently Lg - not have effects, only it is possible to sell it by 456042 Pokéyenes per piece. Another utility for Lg - in Yellow, is get a lot of glitch items via p PKMN p without repeat the Ditto trick process again. When p PKMN p appears and mutate the 5th item, switch the 5th item by other and use Lg - glitch item for restart the battle, when p PKMN p appears again then mutate the 5th item again.

Wow, Paco, I didn't think of that! That really helps, because it was really time-consuming to encounter p PKMN p' ' all of the time.
I was messing around with Lg- today in my Yellow, and it's a very interesting item. In the overworld, using it screws around with the sprites of NPCs, and as Paco pointed out, you can sell it for a huge sum of money (so cloning it is a good idea).
If you use it during a wild Pokemon battle (except for when you are fishing, and when a Pokemon is encountered using the Mew glitch) the battle ends and the screen is messed up (but can be fixed slightly by looking at a Pokemon's status screen, or you can fix it altogether by saving and resetting).
What is interesting, however, is that after this happens, you can actually walk through 1 wall. This works indoors and outdoors. I have used it in the Seafoam Islands to clip past the fast current to get to Articuno early. In outdoor areas, this small walk-through-walls effect is pointless because you can do it through the Safari Zone anyway, but I don't think that the Safari Zone's walk-through-walls glitch works when you enter a building. Could there be a use to this that we haven't seen?

Does this only work if you get into a wild Pokemon battle? If so, we can use it to skip a few badge checks if it disables script checking on that step.

This is similar to the outside battle effects of "RIVAL's" (RB:$A2, Y:$A3), "NAME?" (RB:$A3,Y:$A4), "NICKNAME?" (RB:$A4,Y:$A5) and a few other items. Unfortunately I can't see any method of obtaining these particular items even with item mutation, though my math is bad.
The above items have the same outside of battle effect in both Red/Blue and Yellow, apart from the fact that RIVAL's in Red/Blue brings up a menu almost as if the item used was an evolutionary stone (however 'NOT ABLE' is given even for both Eevee and Nidorino) and choosing one gives an 'It won't have any effect' message, whilst RIVAL's in Yellow can act like a Potion.
Basically they bring up a menu, and once the menu is canceled it is possible to change your direction to clip through one wall and sometimes possibly more. This was named the 'RIVAL's' effect when the ItemDex was being made, because I believe that was the first item index number that had that no-clipping effect in Red/Blue.
The Safari Zone no-clip glitch can be exploited in buildings too actually. For example, you can walk into the Pokémon League gatehouse and bypass the first guard (if you skipped the first badge via the Skip Brock glitch in Red/Blue).

@gary 'M 9 Yep, it's only with wild battles, with the exception of fishing and Mew glitch battles. TorchickensI've seen people do the first-badge skip, but whenever I enter a building while messing around with walk-through-walls, the glitch stops for me.

Please disregard what I posted, regarding entering buildings with the Safari Zone no-clipping glitch, I'm really sorry for not double checking. It was a long time ago since I last tested it with building doors rather than just exit/entrance mats and now I feel a bit guilty. Zachtheglitchbuster's video from 2008 (see below) confused me, though I don't know if I was missing an extra step? (The associated Wiki page unfortunately is not archived). I'm now unsure if there was an emulation error or if the video demonstration was illegitimate.
I performed the glitch and walked to the badge check building to get the same result as you; the no-clipping side effect deactivated after entering the door.
