I've seen 100% accuracy moves miss in Gold/Silver/Crystal, without any accurracy drops, I know the 99.6 glitch has been fixed, so how can this happen?

Are you sure that it wasn't a move which only has 100% in later generations? For example, Tackle has 100% accuracy in Generation V, but prior to this it only had 95% accuracy. Other than trying to hit when the Pokémon is in the air or underground I honestly don't know what to suggest. I do not know how Generation II regards accuracy. In Generation I, a "100%" accuracy move is internally 255/256 (99.6% as you said) because 0/0 === 256/256 is managed as zero accuracy, meaning all moves except Swift (which was made to even hit if you used Dig or Fly until Generation II) have a small chance of missing in Generation I. See also here for a list of internal accuracy values. Bulbapedia says that the bug was fixed in Generation II, though I'm not sure because the G/S 100% moves, starting at $41AFE in the ROM have 255/256 as well (the move data is stored the same like in IIMarckus' list but with an extra 7th byte which is always 0?), so it would have to be a change in how the battles work. Sorry that I couldn't really be of any help. Incidentally, someone needs to look for unused G/S battle effects because R/B had some e.g. +2 Evasiveness as used on the glitch attack TM13. I'll have a look through them.

IIRC, various non-damaging moves have imperfect accuracy, even though the game says otherwise... I think it's even less than 99.6%, too.
